Het WHO-Kaderverdrag inzake Tabaksontmoediging (FCTC) heeft als doel de gezondheid te beschermen door de schade van roken te verminderen. De Conferentie van Partijen (COP) zou wereldwijd tabaksbeleid moeten vaststellen op basis van bewijs en schadebeperking. COP11 brengt echter problemen aan het licht die de doelstellingen bedreigen en negeert wat daadwerkelijk werkt om de schade van roken te verminderen.<\/span><\/p>\n

Hier zijn elf belangrijke kwesties die de vermindering van de schade door tabaksgebruik en de volksgezondheid op COP11 ondermijnen:<\/span><\/p>\n

    \n
  1. Schadebeperking wordt systematisch afgewezen\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

    Schadebeperking is duidelijk gedefinieerd in artikel 1(d) van de FCTC, maar op COP11 wordt het behandeld als een tactiek van de tabaksindustrie. In plaats van evidence-based alternatieven zoals e-vaping en nicotinezakjes te ondersteunen, ontkennen het Secretariaat en vele ngo's de legitimiteit van schadebeperking en blokkeren ze de discussie.<\/span><\/p>\n

      \n
    1. De wettelijke basis voor schadebeperking wordt opzettelijk over het hoofd gezien op COP11\u00a0<\/strong><\/li>\n<\/ol>\n

      Discussies over schadebeperking worden ondergebracht bij agendapunt 4.5, dat betrekking heeft op de implementatie van artikel 5.2(b) van de FCTC. Dat artikel richt zich op hoe overheden hun nationale infrastructuur voor tabaksontmoediging zouden moeten organiseren, en is voornamelijk administratief van aard, waarbij schadebeperking helemaal niet wordt genoemd. Door dit te doen, vermeed het Secretariaat de verwijzing naar artikel 1(d), waarin schadebeperking expliciet wordt erkend als een van de drie pijlers van het verdrag. Deze juridische omkadering stelt COP11 in staat om schadebeperking buiten beschouwing te laten in het evidence-based debat.<\/span><\/p>\n

        \n
      1. Bewijs uit de echte wereld genegeerd\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

        Landen zoals Zweden, het Verenigd Koninkrijk, Japan en Nieuw-Zeeland hebben het roken aanzienlijk teruggedrongen door gereguleerde, minder schadelijke alternatieven mogelijk te maken. Volksgezondheidsautoriteiten en onafhankelijke onderzoeken bevestigen deze resultaten, maar COP11 laat na deze lessen te verwerken.<\/span><\/p>\n

          \n
        1. Verbod als standaardbeleid\u00a0<\/strong>\u00a0<\/span><\/li>\n<\/ol>\n

          Veel COP-voorstellen dringen aan op verboden en beperkingen. De geschiedenis leert dat een verbod averechts werkt doordat rokers naar sigaretten of illegale markten worden gedreven. COP11 negeert deze onbedoelde gevolgen en mist pragmatische benaderingen.<\/span><\/p>\n

            \n
          1. Nicotine zelf is het doelwit\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

            COP11 richt zich op het verminderen van nicotineconsumptie in plaats van op de schade die roken met zich meebrengt. Er wordt echter over het hoofd gezien dat nicotine zonder verbranding veel minder schadelijk is en dat tabaksbestrijding prioriteit zou moeten geven aan het verminderen van dodelijke blootstelling aan rook.<\/span><\/p>\n

              \n
            1. Uitsluiting van onafhankelijke experts COP11\u00a0<\/strong><\/li>\n<\/ol>\n

              Het uitsluiten van experts op het gebied van volksgezondheid met uiteenlopende meningen beperkt de toegang tot onafhankelijke wetenschappers en onderzoekers, wat de kritische evaluatie van bewijsmateriaal vermindert en de erkenning van opkomende strategie\u00ebn voor schadebeperking vertraagt. Door de inbreng van experts te beperken, riskeert het beleidsproces groepsdenken en mist het een genuanceerd wetenschappelijk debat over minder schadelijke producten en de effectiviteit ervan in de praktijk.<\/span><\/p>\n

              1. Transparantie en openheid ontbreken\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                COP11 vindt grotendeels achter gesloten deuren plaats, met complexe registratie en beperkte toegang voor media en publiek. Dit gebrek aan openheid ondermijnt vertrouwen, verantwoording en open debat, die essentieel zijn voor gedegen beleid.<\/span><\/p>\n

                  \n
                1. Artikel 5.3 wordt misbruikt om debat te smoren\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                  Artikel 5.3 is bedoeld om beleidsvorming te beschermen tegen inmenging van de tabaksindustrie. Het wordt gebruikt om onafhankelijke stemmen die zich inzetten voor schadebeperking te blokkeren. Dit wordt gedaan door elke afwijkende mening ervan te beschuldigen dat het een mening van de industrie is.<\/span><\/p>\n

                    \n
                  1. Voorgestelde maatregelen zonder bewijs\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                    Sommige COP-maatregelen op het gebied van milieu, aansprakelijkheid en regelgeving missen solide bewijs voor effectiviteit of analyse van de schadebeperkende effecten. De werkgroepen en nevenevenementen tonen ook een duidelijke vooringenomenheid tegen alternatieve nicotineproducten.<\/span><\/p>\n

                      \n
                    1. Tegenstrijdigheid tussen acties en doelstellingen voor de volksgezondheid\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                      COP11 beweert de gezondheid te beschermen, maar het beleid staat in schril contrast met de daadwerkelijke daling van het roken in landen waar schadebeperkende producten beschikbaar zijn. Het verbod op snus in de EU wordt aangehaald als voorbeeld van een beleid zonder bewijs voor effecten op de volksgezondheid.<\/span><\/p>\n

                        \n
                      1. Ontbrekende consumentenstemmen\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                        Consumenten hebben ervaring met tabaksproducten en schadebeperkende maatregelen. Hun perspectieven worden niet vertegenwoordigd in de COP11-discussies, wat resulteert in beleid dat niet aansluit bij het gebruik, de voorkeuren of de praktische aspecten van de overstap naar minder schadelijke producten.<\/span><\/p>\n

                        COP11 kan doorgaan met het negeren van bewijs en het handhaven van prohibitieve, dogmatische beleidsmaatregelen die schadelijk kunnen zijn voor rokers. Maar het kan ook het terugdringen van de schade door tabak omarmen als een bewezen, ethische en kosteneffectieve manier om aan roken gerelateerde ziekten en sterfgevallen terug te dringen.<\/span><\/p>\n

                        De toekomst van wereldwijde tabaksbestrijding hangt af van transparantie, inclusiviteit en wetenschappelijk onderbouwd beleid, gebaseerd op daadwerkelijke effectiviteit.
