{"id":6935,"date":"2026-04-21T06:35:46","date_gmt":"2026-04-21T10:35:46","guid":{"rendered":"https:\/\/worldvapersalliance.com\/?p=6935"},"modified":"2026-04-21T06:35:46","modified_gmt":"2026-04-21T10:35:46","slug":"former-who-officials-to-global-health-community-harm-reduction-works","status":"publish","type":"post","link":"https:\/\/worldvapersalliance.com\/nl_be\/former-who-officials-to-global-health-community-harm-reduction-works\/","title":{"rendered":"Voormalige WHO-functionarissen tegenover de wereldwijde gezondheidsgemeenschap: schadebeperking werkt"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Drie voormalige WHO-functionarissen hebben een publicatie uitgebracht. <\/span><a href=\"https:\/\/www.nature.com\/articles\/s44360-026-00121-1\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">commentaar in Nature Health<\/span><\/a><span style=\"font-weight: 400;\"> Het artikel roept de wereldwijde gezondheidsgemeenschap op om de vermindering van de schadelijke gevolgen van tabak formeel te integreren in het internationale tabaksbeleid. De auteurs, Robert Beaglehole, Ruth Bonita en Tikki Pang, hebben hun carri\u00e8re gewijd aan het opbouwen van de wereldwijde structuur voor tabaksbestrijding. Wanneer mensen met die achtergrond zeggen dat de huidige aanpak faalt en dat minder schadelijke alternatieven deel uitmaken van de oplossing, is dat belangrijk.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Hun kernargument is eenvoudig. Roken eist nog steeds jaarlijks meer dan zeven miljoen levens. De doelstelling van de WHO om het tabaksgebruik met 301 ton te verminderen tegen 2025 ligt niet op schema. Evenmin wordt de Duurzame Ontwikkelingsdoelstelling gehaald om de vroegtijdige sterfte door niet-overdraagbare ziekten tegen 2030 met een derde te verminderen. De vooruitgang is gestagneerd en de auteurs zijn er duidelijk over waarom: de huidige aanpak kan het aantal rokers niet snel genoeg terugdringen, met name niet onder oudere, meer afhankelijke rokers die al meerdere keren hebben geprobeerd te stoppen en daarin zijn mislukt.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">De auteurs zijn ook openhartig over een probleem dat in de discussies over tabaksbestrijding is geslopen. In sommige beleidskringen is het doel stilletjes verschoven van het elimineren van brandbare tabak naar het volledig elimineren van nicotine. Het commentaar verzet zich hiertegen. Decennia aan bewijs tonen aan dat blootstelling aan rook van verbranding, en niet nicotine, de oorzaak is van tabaksgerelateerde ziekten. Het verwarren van beide beschermt de volksgezondheid niet, maar staat deze juist in de weg.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">De oplossing die de auteurs voorstellen, wijkt niet af van de bestaande tabaksbestrijding, maar vormt er juist een aanvulling op. Ze pleiten voor een wereldwijd rookvrij land in 2040, gedefinieerd als een dagelijkse rookprevalentie onder volwassenen van minder dan 51 ton per 4 ton in 2040, te bereiken door de bestaande maatregelen van het Kaderverdrag inzake tabaksbestrijding te combineren met een bredere toegang tot gereguleerde, rookvrije nicotinealternatieven. Cruciaal is dat ze erop wijzen dat schadebeperking geen nieuw idee is dat van buiten het kader van het FCTC wordt ge\u00efmporteerd. Het wordt al expliciet erkend in artikel 1(d) van het FCTC als een onderdeel van een alomvattende tabaksbestrijding, naast strategie\u00ebn voor het verminderen van vraag en aanbod. Het is in de praktijk echter simpelweg genegeerd.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Het bewijsmateriaal waarnaar ze verwijzen, is hetzelfde bewijsmateriaal dat voorstanders van schadebeperking al jaren aanhalen. Zweden heeft het laagste rookpercentage van Europa en longkankercijfers die minder dan de helft lager liggen dan het EU-gemiddelde, dankzij decennialange toegang tot snus en nicotinezakjes. Japan zag een ongekende daling van de sigarettenverkoop nadat verhitte tabaksproducten op grote schaal beschikbaar kwamen. Nieuw-Zeeland zag de daling van het roken na 2018 sterk versnellen, toen de toegang tot gereguleerde e-sigarettenproducten werd uitgebreid, met de sterkste dalingen onder de Maori en andere historisch achtergestelde groepen.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Wat betreft jongeren zijn de auteurs specifiek. In Nieuw-Zeeland komt regelmatig vapen onder niet-rokers zelden voor, is experimenteren met vapen de afgelopen jaren afgenomen en ligt de prevalentie van roken onder jongeren nu rond de 11 op de 400. De zorg dat vapen jongeren aanzet tot roken, wordt niet ondersteund door gegevens op populatieniveau. Sterke bescherming van jongeren en toegankelijke alternatieven voor volwassen rokers hoeven elkaar niet tegen te spreken.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Het meest treffende deel van het commentaar gaat wellicht over de scheve verhouding tussen regelgeving. Sigaretten blijven in veel markten op grote schaal verkrijgbaar met relatief lichte beperkingen, terwijl minder schadelijke alternatieven te maken krijgen met zwaardere regelgeving, smaakverboden en in sommige gevallen zelfs een regelrecht verbod. De auteurs beschrijven dit als een scheve verhouding die het risico met zich meebrengt dat de gevaarlijkste producten worden beschermd, terwijl de toegang tot minder schadelijke alternatieven wordt beperkt. Een risicoproportioneel kader zou het tegenovergestelde doen: de strengste beperkingen toepassen op brandbare tabak en alternatieven reguleren op basis van hun werkelijke risico.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Deze conclusies zijn afkomstig van mensen die hebben meegewerkt aan het ontwerp van het wereldwijde tabaksbestrijdingssysteem en die geen enkel commercieel belang hebben. Hun conclusie is dat het systeem moet worden gemoderniseerd om schadebeperking te omvatten, en dat het nalaten daarvan levens zal kosten.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">De WHO heeft zich verzet tegen dit standpunt. Tijdens haar bijeenkomsten in het kader van het FCTC is er herhaaldelijk niet serieus gekeken naar het bewijsmateriaal over minder schadelijke alternatieven. Dat verzet heeft gevolgen. Elk jaar dat de WHO de overstap uitstelt, stappen meer rokers die dat hadden kunnen doen, niet over. Het commentaar in Nature Health is een directe uitdaging van dat standpunt, afkomstig van mensen die de WHO niet zomaar terzijde kan schuiven.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Het bewijs is er. De praktijkvoorbeelden zijn er. Wat de auteurs echter missen, is politieke wil.
