Het WHO-Raamverdrag inzake tabaksbestrijding (FCTC) heeft als doel de gezondheid te beschermen door de schadelijke gevolgen van roken te verminderen. De Conferentie van Partijen (COP) zou een wereldwijd tabaksbeleid moeten vaststellen op basis van bewijs en schadebeperking. COP11 onthult echter problemen die de doelstellingen bedreigen en negeert wat effectief is om de schadelijke gevolgen van roken te verminderen.<\/span><\/p>\n

Hieronder volgen elf belangrijke punten die de vermindering van tabaksschade en de volksgezondheid op COP11 ondermijnen:<\/span><\/p>\n

    \n
  1. Schadebeperking wordt systematisch afgewezen.\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

    Schadebeperking is duidelijk gedefinieerd in artikel 1(d) van het FCTC, maar op COP11 wordt het behandeld als een tactiek van de tabaksindustrie. In plaats van op bewijs gebaseerde alternatieven zoals e-sigaretten en nicotinezakjes te ondersteunen, ontkennen het secretariaat en veel ngo's de legitimiteit van schadebeperking en blokkeren ze de discussie.<\/span><\/p>\n

      \n
    1. De juridische basis voor schadebeperking wordt opzettelijk over het hoofd gezien tijdens COP11.\u00a0<\/strong><\/li>\n<\/ol>\n

      Discussies over schadebeperking worden ondergebracht onder agendapunt 4.5, dat handelt over de implementatie van artikel 5.2(b) van het FCTC. Dat artikel richt zich op de wijze waarop overheden hun nationale infrastructuur voor tabaksbestrijding moeten organiseren en is voornamelijk administratief van aard, zonder schadebeperking ook maar enigszins te noemen. Door dit te doen, vermeed het secretariaat het verwijzen naar artikel 1(d), waarin schadebeperking expliciet wordt erkend als een van de drie pijlers van het verdrag. Deze juridische inkadering stelt COP11 in staat om schadebeperking buiten het op bewijs gebaseerde debat te houden.<\/span><\/p>\n

        \n
      1. Bewijs uit de praktijk genegeerd\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

        Landen zoals Zweden, het Verenigd Koninkrijk, Japan en Nieuw-Zeeland hebben het roken aanzienlijk teruggedrongen door gereguleerde, minder schadelijke alternatieven mogelijk te maken. Volksgezondheidsautoriteiten en onafhankelijke onderzoeken bevestigen deze resultaten, maar COP11 besteedt geen aandacht aan deze lessen.<\/span><\/p>\n

          \n
        1. Verbod als standaardbeleid\u00a0<\/strong>\u00a0<\/span><\/li>\n<\/ol>\n

          Veel COP-voorstellen pleiten voor verboden en beperkingen. De geschiedenis leert dat een verbod averechts werkt en rokers naar sigaretten of illegale markten drijft. COP11 negeert deze onbedoelde gevolgen en laat pragmatische benaderingen buiten beschouwing.<\/span><\/p>\n

            \n
          1. Nicotine zelf is het doelwit.\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

            COP11 richt zich op het verminderen van nicotinegebruik in plaats van op de schade die roken veroorzaakt. Het negeert het feit dat nicotine zonder verbranding veel minder schadelijk is en dat tabaksbestrijding prioriteit moet geven aan het verminderen van de dodelijke blootstelling aan rook.<\/span><\/p>\n

            1. Uitsluiting van onafhankelijke deskundigen COP11\u00a0<\/strong><\/li>\n<\/ol>\n

              Het uitsluiten van volksgezondheidsdeskundigen met afwijkende meningen beperkt de toegang tot onafhankelijke wetenschappers en onderzoekers, wat de kritische evaluatie van bewijsmateriaal belemmert en de erkenning van nieuwe strategie\u00ebn voor schadebeperking vertraagt. Door de inbreng van experts te beperken, dreigt er groepsdenken in het beleidsproces en wordt een genuanceerd wetenschappelijk debat over minder schadelijke producten en hun effectiviteit in de praktijk gemist.<\/span><\/p>\n

                \n
              1. Transparantie en openheid ontbreken.\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                COP11 vindt grotendeels achter gesloten deuren plaats, met een complex registratieproces en beperkte toegang voor de media en het publiek. Dit gebrek aan openheid ondermijnt het vertrouwen, de verantwoordingsplicht en het open debat, die essentieel zijn voor een degelijk beleid.<\/span><\/p>\n

                1. Artikel 5.3 wordt misbruikt om het debat te smoren.\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                  Artikel 5.3 is ontworpen om de beleidsvorming te beschermen tegen inmenging van de tabaksindustrie, maar wordt misbruikt om onafhankelijke stemmen die pleiten voor schadebeperking de mond te snoeren door elke afwijkende mening ervan te beschuldigen een mening van de industrie te zijn.<\/span><\/p>\n

                    \n
                  1. Voorgestelde maatregelen waarvoor geen bewijs is\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                    Sommige COP-maatregelen op het gebied van milieu, aansprakelijkheid en regelgeving missen solide bewijs van effectiviteit of analyse van de effecten op de vermindering van schadelijke gevolgen. De werkgroepen en nevenevenementen tonen bovendien een duidelijke vooringenomenheid tegen alternatieve nicotineproducten.<\/span><\/p>\n

                      \n
                    1. Tegenstrijdigheid tussen acties en doelstellingen voor de volksgezondheid\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                      COP11 beweert de gezondheid te beschermen, maar het beleid staat haaks op de daadwerkelijke afname van roken in gebieden waar producten ter vermindering van de schadelijke gevolgen beschikbaar zijn. Het EU-verbod op snus wordt aangehaald als voorbeeld van een beleid zonder aantoonbare resultaten voor de volksgezondheid.<\/span><\/p>\n

                      1. Ontbrekende stemmen van consumenten\u00a0\u00a0<\/strong><\/li>\n<\/ol>\n

                        Consumenten hebben ervaring met tabaksproducten en methoden om de schade te beperken. Hun perspectieven worden niet vertegenwoordigd in de discussies van COP11, wat resulteert in beleid dat geen rekening houdt met het daadwerkelijke gebruik, de voorkeuren of de praktische aspecten van de overstap naar minder schadelijke producten.<\/span><\/p>\n

                        COP11 kan bewijs blijven negeren en dogmatische, verbodsbepalingen blijven handhaven die het risico met zich meebrengen dat rokers erdoor worden geschaad, of het kan tabakschadebeperking omarmen als een bewezen, ethische en kosteneffectieve manier om ziekte en sterfte als gevolg van roken te verminderen.<\/span><\/p>\n

                        De toekomst van de wereldwijde tabaksbestrijding hangt af van transparantie, inclusiviteit en wetenschappelijk onderbouwde beleidsmaatregelen die gebaseerd zijn op effectiviteit in de praktijk.
