I verden af skademinimering af tobak f\u00f8lger mindre stater ofte eksemplerne fra st\u00f8rre lande eller institutioner. Selvom de har mindre befolkninger, er de beslutninger om folkesundhed, disse stater tr\u00e6ffer, ikke mindre vigtige.\u00a0<\/span><\/p>\n

Problemet er stort set det samme overalt \u2013 hvad er den bedste m\u00e5de at reducere rygerater p\u00e5, og hvordan kan forbrugerne hj\u00e6lpes p\u00e5 deres rejse mod at holde op?<\/span><\/p>\n

Desv\u00e6rre er debatten om nikotin stadig forskellig rundt om i verden. Vi ser en tendens til at d\u00e6monisere nikotin og tilskrive skaderne forbundet med traditionelle cigaretter og forbr\u00e6nding til selve nikotinen. Et af de vigtigste principper for reduktion af tobaksskader er dog, at ikke alle nikotinprodukter indeb\u00e6rer samme risikoniveau.\u00a0<\/span><\/p>\n

Et godt eksempel p\u00e5, hvordan politikudformningen omkring tobak kan v\u00e6re uforenelig med videnskaben i mindre stater, kan ses i<\/span> adressen <\/span><\/a>af Palaus pr\u00e6sident Surangel Whipps til FN. I den f\u00f8rste del af sin tale talte han om vigtigheden af videnskabelig evidens i politikudformningen. Senere undlod han dog at anerkende den videnskabelige evidens omkring nikotin.\u00a0<\/span><\/p>\n

Palau har formelt anmodet FN's generalsekret\u00e6r om at iv\u00e6rks\u00e6tte en WHO-gennemgang af nikotin som et psykotropisk stof. De, der er bekendt med WHO's tilgang til mindre skadelige nikotinalternativer, ville ikke forvente nogen positiv udvikling for reduktion af tobaksskader som f\u00f8lge af dette.\u00a0<\/span><\/p>\n

WHO har gentagne gange afvist de videnskabelige beviser, der underst\u00f8tter mindre skadelige nikotinalternativer som v\u00e6rkt\u00f8jer til rygestop, og opfordret lande til at indf\u00f8re forbudspolitikker over for innovative produkter. WHO's indflydelse kan v\u00e6re s\u00e6rligt betydelig i mindre lande, s\u00e5som Palau.\u00a0<\/span><\/p>\n

I sin tale udtalte pr\u00e6sidenten endda, at "nikotin kr\u00e6ver over 7 millioner liv \u00e5rligt" og opfordrede til st\u00e6rkere politikker for at reducere nikotinafh\u00e6ngighed, is\u00e6r blandt unge.\u00a0<\/span><\/p>\n

Denne formulering er problematisk, fordi de syv millioner \u00e5rlige d\u00f8dsfald, der ofte n\u00e6vnes i denne debat, er forbundet med tobaksbrug, drevet af skadevirkningerne ved rygning og eksponering for tobaksr\u00f8g. Nikotin er vanedannende, men det er<\/span> ikke hoved\u00e5rsagen<\/span><\/a> mennesker d\u00f8r af rygning.<\/span>

Hvorfor er denne forskel vigtig?<\/span><\/p>\n

Mindre risikable nikotinprodukter kan g\u00f8re det lettere for voksne rygere at holde op. Vi har set denne model f\u00e5 succes i Sverige, som har bragt sin rygeprocent ned. <\/span>under 5%<\/span><\/a> og blev det f\u00f8rste r\u00f8gfri land i EU, hvor mindre skadelige produkter s\u00e5som poser spillede en vigtig rolle.\u00a0<\/span><\/p>\n

Vigtigt er det, at en opdateret <\/span>Cochrane-anmeldelsen <\/span><\/a>Ud af 80 randomiserede fors\u00f8g, der d\u00e6kker n\u00e6sten 30.000 rygere, har man fundet ud af, at nikotindampning hj\u00e6lper flere rygere med at holde op end nikotinudskiftningsterapi (NRT) s\u00e5som tyggegummi eller plaster. Cochrane vurderer resultatet som "h\u00f8j sikkerhed" (den h\u00f8jeste karakter, som de giver, og som de sj\u00e6ldent giver). Omkring fire flere personer ud af hundrede holder op med nikotindampning end med plaster eller tyggegummi. Man s\u00e5 heller ingen tegn p\u00e5 \u00f8get alvorlig skade, og man fandt, at nikotindampning virker bedre end nikotinfri.<\/span><\/p>\n

Med andre ord, mens regeringer b\u00f8r arbejde for at forhindre unges optagelse gennem foranstaltninger som strenge aldersgr\u00e6nser, b\u00f8r de ikke ignorere potentialet i mindre skadelige alternativer for voksne, der allerede ryger.\u00a0<\/span><\/p>\n

I mods\u00e6tning hertil tog en anden lille \u00f8stat en <\/span>anderledes tilgang <\/span><\/a>Ved COP11 pressede Saint Kitts og Nevis p\u00e5 for en dedikeret diskussion om skadereduktion fra tobak inden for WHO's FCTC, forsvarede deres ret til at vedtage politikker, der afspejler deres egen virkelighed, og st\u00f8ttede videnskabsbaserede politikker og skadereduktion.<\/span><\/p>\n

Mindre stater b\u00f8r kunne udvikle tobakspolitikker, der afspejler deres egne folkesundhedsbehov og den fulde m\u00e6ngde af tilg\u00e6ngelig videnskabelig evidens, i stedet for at behandle alle nikotinprodukter som ligev\u00e6rdige med br\u00e6ndbare cigaretter.
