Forfatterne, Robert Beaglehole, Ruth Bonita og Tikki Pang, har i deres karriere opbygget den globale arkitektur for tobakskontrol. Afg\u00f8rende er det, at de p\u00e5peger, at skadesreduktion ikke er en ny id\u00e9, der importeres uden for rammerne af FCTC. Japan oplevede hidtil usete fald i cigaretsalget, efter at opvarmede tobaksvarer blev bredt tilg\u00e6ngelige. New Zealand accelererede sit rygefald kraftigt efter 2018, hvor adgangen til regulerede vapingprodukter blev udvidet, med de st\u00f8rste fald blandt maorier og andre historisk d\u00e5rligt stillede grupper.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Forfatterne er specifikke om unge. I New Zealand er regelm\u00e6ssig vaping blandt aldrig-rygere sj\u00e6lden, eksperimentering er faldet i de senere \u00e5r, og pr\u00e6valensen af unges rygning er nu omkring 1%. Bekymringen bag indgangen, ideen om at vaping f\u00f8rer unge til rygning, underst\u00f8ttes ikke af data p\u00e5 befolkningsniveau. St\u00e6rk ungdomsbeskyttelse og tilg\u00e6ngelige alternativer for voksne rygere er ikke i konflikt.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Den m\u00e5ske mest skarpe del af kommentaren omhandler regulatorisk uoverensstemmelse. Cigaretter er fortsat bredt tilg\u00e6ngelige med relativt lette restriktioner p\u00e5 mange markeder, mens mindre skadelige alternativer st\u00e5r over for tungere regulatoriske byrder, smagsforbud og i nogle tilf\u00e6lde direkte forbud. Forfatterne beskriver dette som en uoverensstemmelse, der risikerer at beskytte de farligste produkter, samtidig med at adgangen til mindre skadelige erstatninger begr\u00e6nses. En risikoproportional ramme ville g\u00f8re det modsatte: anvende de strengeste restriktioner p\u00e5 br\u00e6ndbar tobak og regulere alternativer baseret p\u00e5 deres faktiske risiko.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Disse konklusioner kommer fra folk, der har hjulpet med at designe det globale system til tobakskontrol, og som ikke har nogen interesse i noget kommercielt resultat. Deres konklusion er, at systemet skal opdateres, s\u00e5 det ogs\u00e5 inkluderer skadesreduktion, og at det vil koste liv, hvis dette ikke g\u00f8res.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">WHO har v\u00e6ret modstander af denne holdning.
